What If Trauma Is Not an Individual Burden?

          But a Collective Response

When we talk about trauma, we often imagine it as an internal wound—something hidden in the mind or body of the individual who suffered it. In Western psychology especially, trauma has long been treated as a

          personal problem

: a set of symptoms to be managed, medicated, or, at best, “worked through” in therapy.

But what if that view is incomplete? What if trauma is not just a private burden, but something that lives in our relationships, our communities, and even in the way our societies are structured?

Trauma on a Continuum of Human Experience

The more deeply we study trauma, the clearer it becomes that people living with psychiatric symptoms are not a separate category of humanity. They are not “other.” Instead, they live further along a long continuum of traumatic adaptation that chains us all together.

From soldiers who return from war, to children who grow up with neglect, to ordinary people navigating the daily pressures of survival—trauma does not neatly divide the “well” from the “ill.” It’s part of our common, and often tragic, humanity.

It’s Not Just the Trauma, It’s the Response

Here’s the striking truth: the trauma itself does not determine the outcome. What matters more is the

response of the group around the person

          .

A supportive community can buffer the effects of trauma, offering safety, belonging, and meaning. An indifferent or punitive environment, on the other hand, can push the person toward isolation, psychiatric breakdown, or even cycles of criminal behavior.

This insight flips the usual question on its head. Instead of asking,

          “What’s wrong with this person?”

           we might ask,

“What’s happened to them—and how have we responded?”

          Rethinking Attachment and Dependency

In Western philosophy and psychology, attachment behaviors are often treated with suspicion. Neediness, dependency, regression, manipulation—these are the labels given to people who reach out for connection when they are hurting.

But what if we saw these behaviors not as signs of weakness, but as survival strategies? In many cultural traditions, interdependence is not pathological—it is human. To lean on others in times of distress is not failure. It is wisdom.

Shifting the Lens From the Individual to the Collective

If trauma outcomes depend on the group response, then our responsibility extends beyond the therapy room. Healing is not only about the resilience of individuals—it is about the resilience of families, workplaces, schools, and societies.

This means that how we design our communities matters. Do we create systems that isolate people in their pain? Or do we build structures of care, compassion, and accountability that allow healing to unfold?

A Question for All of Us

Perhaps the most radical idea is this: sometimes, the pathology is not in the person who suffers, but in the society that isolates them.

          So the question becomes—

How can we, as individuals and communities, respond differently to trauma so that healing is possible?

          ✨

          Takeaway:

Trauma is not simply an internal scar. It is a relational wound. And if that’s true, then healing can only happen when we recognize that responsibility is shared.

The Broken Pieces I Couldn’t Explain

For most of my life, I thought I was broken. There were days — though I didn’t realise it at the time — that always seemed to come just before my period, when I felt like I was spiralling out of control. I would go from 0 to 100 emotionally — rage, sadness, anxiety, confusion — often all at once. The smallest thing would set me off.

My husband, who’s stood by me for 11 years, copped the worst of it. Even before we got married, he told me he saw behaviours that made him question whether we’d last. I couldn’t even blame him — because I didn’t understand myself either.

          Where It Began

I didn’t have an easy start in life. My mum, who I love dearly, struggled with alcohol when she was pregnant with me. I was born into dysfunction, surrounded by violence and abuse. By my teens and early 20s, I was in a spiral — drugs, gangs, toxic relationships, arrests. I still remember the day a judge called me a horrible person. I broke down and said,

“I’m not a bad person. I don’t mean to do these things.”

That was the truth. Even when I was angry, reckless, or acting out… I never wanted to be that way.

✨ Trying to Heal and Grow

From 14 to 24, I worked and studied. I got into social work. I began to discover who I was as an Indigenous woman and a Christian. That spiritual and cultural grounding helped shape the next 20 years of my life.

I became a mum. A wife. I built a life that didn’t reflect where I came from. I practiced self-care, therapy, journaling, prayer, exercise, eating well. I strived for emotional stability.

But no matter how much work I did…
No matter how balanced my life became…
Something still came for me

          every month

           — and it took me out.

           The Monthly Crash

Each month, I would hit a low so deep it didn’t make sense. Some months, I couldn’t control my outbursts. I was reactive, sensitive, irrational. I needed love and attention — but didn’t know how to ask for it. So instead, I’d lash out. I became negative or overly emotional — the complete opposite of how I wanted to live.

The lack of understanding — from me, from others — made things worse.
I’d spiral into emotional situations I couldn’t pull myself out of.
I self-harmed in silence, just to release the pressure.
Once, while pregnant, I reached a point so dark, I thought about ending it all.

And still, no one could tell me

          why

          .

           What Is PMDD?

Eventually, my husband and I started digging. And then… I found the name:

          PMDD — Premenstrual Dysphoric Disorder.

          PMDD is not “just PMS.”
It’s a severe, hormone-based mood disorder.
It causes extreme shifts in mood, energy, and emotional control — usually in the 1–2 weeks before your period (the luteal phase).

          According to research, about

          5–8% of women

experience PMDD, though many go undiagnosed for

          years

. The symptoms mimic depression or anxiety, and are often dismissed as being “too emotional” or “dramatic.”

          Common symptoms include:

           Irritability, rage, and sudden sadness

           Feeling out of control

Panic, hopelessness, or even suicidal thoughts

           Relationship conflict and social withdrawal

           Fatigue, insomnia, or physical pain

And what makes it worse? These symptoms disappear after your period. Which makes you question your own reality every month

Finally having a name for what I was experiencing changed everything.

It didn’t “fix” me overnight. But it gave me

          language

          . It gave my husband

          understanding

          . It gave us both

          compassion

Now we’re exploring treatment options, therapy tools, and ways to plan around

          that week

. It’s still hard. But now I know it’s not my personality — it’s PMDD. And I’m not alone.

           If You’ve Felt This Too…

If you’ve ever hated yourself for what you said before your period…
If you’ve ever felt broken, dismissed, or misunderstood…
If you’ve ever thought,

          “Why am I like this?”

           — I want you to know:

You’re not broken. You’re not alone. And you’re not imagining it.

Start tracking your symptoms. Talk to a doctor who will listen. Look into PMDD.
Because awareness is the first step toward healing.

I wrote this for every woman still surviving when she JUST wants to thrive.

Let’s break the silence. Let’s name it.
Because it’s real. And we deserve better.

PMDD and the NDIS: Should It Be Recognised and Supported?

          What Is PMDD?

Premenstrual Dysphoric Disorder (PMDD) is a hormone-based mood disorder that affects approximately 5–8% of menstruating individuals. Often misunderstood or mistaken for severe PMS (premenstrual syndrome), PMDD is much more serious in both its presentation and its impact.

Symptoms of PMDD typically arise during the luteal phase of the menstrual cycle (the week before menstruation) and disappear within a few days after menstruation begins. These symptoms include:

           Intense mood swings

           Severe irritability or anger

           Debilitating depression or hopelessness

           Anxiety or panic attacks

           Fatigue and low energy

           Sleep disturbances

           Brain fog or poor concentration

Physical symptoms like breast tenderness, bloating, and joint pain

For many, these symptoms are not just unpleasant — they are life-altering. Individuals with PMDD can struggle with basic daily tasks, relationships, employment, education, and self-care. In extreme cases, PMDD can lead to suicidal ideation.

          The Daily Impact of PMDD

Though it only affects part of each month, PMDD’s impact is profound. Many sufferers describe losing half their month to symptoms and spending the other half recovering from the fallout. This repeated, cyclical impairment can severely disrupt lives over time, often leading to job loss, relationship breakdowns, isolation, and comorbid mental health conditions such as anxiety or depression.

Because PMDD is not widely understood — even among healthcare providers — many individuals go undiagnosed or receive inappropriate treatment for years. This leads to feelings of invalidation and hopelessness.

Why PMDD Should Be Recognised by the NDIS

The National Disability Insurance Scheme (NDIS) in Australia is designed to support individuals with a permanent and significant disability. Although PMDD is not currently listed as a primary disability under the scheme, there is a strong case for its inclusion, especially under the psychosocial disability category.

          Here’s why:

           Functional Impairment:

PMDD leads to severe functional impairment in multiple areas, including emotional regulation, work participation, social interaction, and personal care — all domains relevant to NDIS access.

           Chronic and Recurrent:

While PMDD symptoms are cyclical, they are consistent and chronic across many years — often decades. This repeated pattern of impairment is equivalent to other recognized psychosocial disabilities.

           Psychiatric Recognition:

PMDD is listed in the DSM-5, the primary diagnostic guide for mental health conditions, and can co-occur with depression, anxiety, ADHD, and trauma disorders. Recognition in psychiatric frameworks adds weight to its legitimacy as a disabling condition.

           Access to Stability:

NDIS support can provide access to therapy, community supports, medication management, and even employment support — all of which could significantly improve quality of life for individuals with PMDD.

Why PMDD Might Not Be Included (Yet)

Despite these strong arguments, there are reasons why PMDD is not currently recognised under the NDIS:

           Cyclical Nature:

Because PMDD symptoms occur during specific times of the month, it may not meet the NDIS’s definition of a

disability — even though the condition is chronic.

           Under-Recognition:

PMDD is still under-recognised by the medical community, which makes it less likely to be formally acknowledged by national schemes.

           Lack of Data and Advocacy:

Unlike better-known disorders, there is a lack of widespread advocacy, funding, and research on the disability impact of PMDD.

           Inconsistent Diagnosis:

Not all GPs and mental health professionals are trained to diagnose PMDD, making it harder for people to gather the required documentation for support applications.

          What Needs to Change

To pave the way for NDIS inclusion, PMDD needs greater awareness, advocacy, and data collection. The lived experience of individuals must be acknowledged as valid evidence of disability. We also need more mental health professionals who are trained to identify and manage the disorder effectively.

Furthermore, the NDIS framework should evolve to recognise

          cyclical but long-term disabilities

           — because a condition doesn't have to be constant to be life-altering.

          Final Thoughts

PMDD is a serious, misunderstood condition that drastically reduces quality of life for many. While it may not yet be formally recognised by the NDIS, that does not mean it shouldn’t be. With increasing advocacy and awareness, there is potential for PMDD to be supported under the psychosocial disability stream.

Until then, individuals affected by PMDD are encouraged to:

           Seek medical and psychological support

Track symptoms to build evidence of functional impairment

Advocate for recognition of their needs within healthcare and support services

Recognition through the NDIS could change lives. It’s time to start having that conversation.

Understanding the Impact of Stigma on Mental Health

Over the years I have worked as a mental health support worker, I've witnessed how stigma can profoundly affect the lives of those facing mental health challenges. Many individuals I supported were not defined by their diagnoses; they were ordinary people who once held meaningful jobs, nurtured relationships, and lived comfortably within their communities. However, the burden of stigma often overshadowed their identities, resulting in isolation, lost opportunities, and a diminished sense of self-worth.

The 'Like Minds, Like Mine' programme, launched in New Zealand in 1997, aimed to address this very issue by reducing stigma and discrimination against people with mental illness. According to research, over half of the participants in the program reported seeing improvements in discrimination over five years. However, a staggering 89% of respondents still experienced unfair treatment due to their mental health conditions. Many concealed their struggles or avoided job applications, fearing discrimination. The primary source of both positive and negative discrimination often came from those closest to them—their families.

The Subtle Stigma in Healthcare Settings

One of my most impactful experiences was observing how stigma manifested in clinical settings. When accompanying participants to their doctor's appointments, I noticed that some healthcare professionals would often address me, the support worker, rather than the individuals themselves. It was as if the person sitting before them was invisible, incapable of understanding or communicating. This undermined the participant’s autonomy and reinforced a negative self-perception.

Research supports this observation. A study by Henderson et al. (2014) found that mental health-related stigma is pervasive even among healthcare professionals. While many mental health workers advocate for the rights of individuals, unconscious biases often lead to microaggressions and patronising behaviour, which can severely impact recovery and self-efficacy. The document titled "Mental Health-Related Stigma in Health Care and Mental Health-Care Settings" highlights how stigma within healthcare settings can affect the quality of care and how addressing these biases through education and advocacy can lead to better outcomes for mental health service users.

          Moving Away from Institutionalized Mindsets

The transition from institutionalised mental health care to community-based living was a significant step forward. However, old mindsets often linger. I encountered residential settings where participants were treated as if growth and independence were out of reach. Staff would say, “There is no changing them,” or “They will never be independent”. These sentiments were not just limiting but were a form of institutionalised stigma that perpetuated dependency.

When I managed a residential home, I aimed to shift this mindset. The house accommodated individuals aged 30 to 65 with a variety of mental health conditions, such as schizophrenia, OCD, ADHD, paranoid schizophrenia, and depression. Many participants relied on staff for daily tasks, not because they couldn't do them, but because they had been conditioned to believe they couldn't. It was a cycle of learned helplessness reinforced by well-meaning but misguided support.

          A Story of Empowerment

One story that stands out involves a participant with severe anxiety and paranoid schizophrenia. They relied heavily on PRN (as-needed) medication, requesting it almost every other day. I began working with them on small, manageable goals, like learning to do their own laundry. Initially, the fear of being locked in the laundry room (due to a lock on the door) was a significant barrier. By simply removing the lock and offering consistent encouragement, this person gradually became comfortable using the washing machine independently.

Over time, not only did their practical skills improve, but their anxiety decreased noticeably. The need for PRN medication dropped to zero over a year. This transformation wasn't just about learning to do laundry—it was about reclaiming agency, building confidence, and breaking free from the internalised stigma that they were incapable of growth.

The Path Forward: Reducing Stigma through Empowerment

Breaking the cycle of stigma requires a shift from caretaking to empowerment. It involves seeing people not just through the lens of their diagnosis but as individuals with the potential to learn, adapt, and thrive. Studies have shown that involving individuals with lived experiences in public health campaigns, as seen in the 'Like Minds, Like Mine ' program, can significantly reduce stigma and change public attitudes.

It is crucial to create environments where individuals feel safe, valued, and capable. This involves not only educating healthcare professionals on the importance of treating individuals with respect but also building community programs that promote independence and celebrate progress, no matter how small.

          Conclusion: A Call for Change

The journey to reducing mental health stigma is ongoing. We can make meaningful strides by sharing stories, challenging old mindsets, and promoting inclusive and growth environments. The success of initiatives like 'Like Minds, Like Mine' demonstrates that change is possible when those with lived experience are given a platform and support systems focus on building independence rather than maintaining dependency.

Let us all strive to be allies, not just in word but in action—working together to create a world where mental health challenges do not define an individual’s worth or potential.

The Hidden Mental Health Crisis in Aged Care

Transitioning into aged care is a significant life event that can bring about emotional distress, anxiety, and depression. Studies indicate that over 50% of aged care residents experience mental health issues—a much higher rate than the general older population. The shift from independent living to a structured care environment often results in feelings of loss, social isolation, and emotional distress.

The process of moving into residential aged care involves numerous adjustments. Loss of independence, changes in daily routines, separation from familiar surroundings, and limited social engagement can exacerbate mental health concerns. Many older adults experience stress, grief, and uncertainty as they navigate their new living arrangements. Without adequate emotional and psychological support, these challenges can significantly impact their well-being.

These programs focus on gradual adaptation, community integration, and personal empowerment, leading to better mental health outcomes. Many participants in transitional care have shown increased functional independence, and a significant proportion have even been able to return to the community rather than remain in residential aged care.
